Data Collection Forms for Functional Behavioral Assessments and Behavior Intervention Plans
ABC Forms are used to record types of behaviors observed and the events that precede and follow the behavior. The information gathered in an ABC analysis helps identify events that are maintaining the challenging behavior, appropriate behaviors that are not reinforced, social skills that need to be learned, and environmental conditions that need modification.

Event recording is a process for documenting the number of times a behavior occurs. An observer using event recording makes a tally mark or documents in some way each time a student engages in a target behavior. The observer also records the time period in which the behavior is being observed.

The interval form is used to divide observation period into a number of smaller time periods or intervals, observes the student throughout each interval, and then records whether the behavior occurred or not in that interval.

The purpose of the Interviews is to collect information about events that influence problem behavior. It helps to identify settings, events, and activities that can be targeted through direct observation.

Rating scales are used to count the number of times a behavior occurs in a specific time period. A rate measure should be used if the length of observation time varies from day to day.

Duration Recording documents the length of a behavior by recording the time the behavior begins and ends. Use this if your primary concern is the length of time the student engages in the behavior and the behavior has a clear beginning and end.

Behavior Intervention Plan is a specific plan of action that designs effective positive behavior interventions to teach the student more acceptable behavior(s) to replace the inappropriate behavior. The purpose is to teach the student more appropriate behaviors that meet the same function, or purpose, as the inappropriate behaviors previously exhibited.
